At Hiraeth House, our six‑bedded residential care home, we offer 24/7 support from skilled Mental Health Support Workers, led by an experienced Registered Manager and RMN. Our ethos is to create a safe, therapeutic environment that promotes independence, manages risks effectively, and helps individuals reintegrate into the community—relieving pressures on hospital services.
Through Thrive at Home, we deliver tailored community‑based support designed to move beyond traditional domiciliary care. Our vision is simple: every person has the right to live their best life in the community, supported by recovery‑focused care and meaningful occupation. Alongside bespoke support packages, we also run inclusive social groups to meet wider social and emotional needs.
Responsibilities:
- Promoting service users wellbeing, dignity, and independence
- Encouraging community integration and social activities
- Supporting service users with activities of daily living and to find meaningful occupation, hobbies, voluntary or paid work
- Monitoring any changes in mental state and physical health
- Promoting a healthy, balanced lifestyle
- Supporting service users to attend appointments
- Monitoring and administering medication (mandatory training provided and required)
- Minimal to zero personal care
Shift Patterns:
Hiraeth House:
- Early: 7:00am – 2:30pm
- Late: 1:30pm – 9:00pm
- Nights: 8:30pm – 7:30am
Thrive at Home: Flexible hours between 7am–9pm, 7 days a week, depending on individual care packages.
