Empower Family Group is seeking compassionate and dedicated individuals to join our team as Supported Living Support Workers. In this role, you will support young people aged 16–18 as they transition into adulthood and independent living. You will provide practical, emotional, and safeguarding support within a nurturing and structured environment that promotes wellbeing, development, and independence.

Key Responsibilities

  • Provide Individualised Support: Deliver high-quality, person-centred care tailored to each young person’s support and pathway plans, ensuring their needs and goals are met.
  • Promote Independent Living Skills: Support young people in developing essential life skills such as budgeting, cooking, cleaning, and managing a tenancy, helping them prepare for independent adulthood.
  • Support Educational and Employment Goals: Encourage and assist young people in accessing, reengaging with, or maintaining education, training, or employment opportunities.
  • Safeguarding and Risk Management: Remain vigilant to any safeguarding concerns, follow established protocols, and report issues promptly to ensure the safety and wellbeing of all young people.
  • Build and Maintain Positive Relationships: Establish professional, trusting relationships with young people, acting as a positive role model while maintaining appropriate boundaries.
  • Provide Emotional and Behavioural Support: Offer guidance and support to help young people manage their emotions, behaviours, and mental health, including during times of crisis.
  • Maintain Accurate Records: Complete daily logs, incident reports, monthly summaries, keywork session notes, risk assessments, and support plans in a timely and accurate manner.
  • Collaborate with External Agencies: Work effectively with social workers, education providers, mental health professionals, and other stakeholders to ensure cohesive support for each young person.
  • Encourage Healthy Routines: Assist young people in planning and maintaining structured daily routines that promote physical and emotional wellbeing.
  • Respond to Crisis Situations: Manage challenging behaviour and crisis situations calmly and effectively, using approved de-escalation techniques and ensuring safety.

  • Level 3 Diploma in Children & Young People or Health & Social Care
  • Experience in supported living, semi-independent, or residential care settings
  • Knowledge of Leaving Care framework and Pathway Plans
  • Experience supporting individuals with complex needs
  • Understanding of safeguarding, child protection, and risk management
