We are looking for an exceptional individual to join our ‘eMed UK Clinical Operations’ Team in London. The successful candidate will work alongside the Medical Director, Complex Care Lead and the ‘Proactive Care Group’ to support the delivery of proactive care to our 110,000+ patients across London. eMed GP at Hand aims to put the most accessible, high quality, safe and effective NHS GP service into the hands of every person who chooses to register with us. Powered by pioneering technology, our ‘digital first’ practice is shaping the future of NHS Primary Care, while also offering all the care you would expect from a practice which is part of the NHS family. Please note: This is a part-time role - 20hrs a week across Monday to Friday.
WHAT YOU’LL WORK ON
- Work with the Proactive Care Group on the delivery of proactive care targets for our patients. This includes the Quality & Outcomes Framework (QOF), local and directed enhanced services (LES & DES), the Investment & Impact Fund (IIF) as well as other internal proactive care initiatives as agreed by the proactive care group.
- Accessing relevant Systems (SystmOne, AccuRx etc.).
- Designing, carrying out and running SystmOne searches to identify target cohorts.
- Where appropriate and as agreed with the clinical team, reviewing, amending, correcting or adding clinical codes in relation to proactive care interventions carried out.
- Using email, sms or telephone or other systems to engage with patients in proactive care activities and interventions.
- Liaise with GP at Hand Medicines Management Team on the smooth running of campaigns and management of Campaign responses.
- Escalating any clinical queries encountered in the pursuit of this to an appropriate member of the clinical team.
- On request of the clinical team, accessing relevant IT systems to gather, evaluate and present proactive care related data to the team.
- Work with the Clinical Operations team on maintaining an accurate patient list.
- Where appropriate and as agreed with the clinical team, creating and running searches to identify patients who may no longer reside in the UK and action de-registrations in line with PCSE guidance and in accordance with the GMS contract.
- Participate in data quality initiatives to improve accuracy of clinical information recorded on SystmOne, particularly in relation to proactive care interventions and chronic disease registers.
- Management of the dedicated GPAH Failsafe Screening NHS Mailbox in line with agreed principles.
- Responding to patient queries in a friendly and timely manner.
- Ensuring that all outstanding tasks have been responded to at the end of each week.
- Where appropriate and as directed by the clinical team, accessing a patient's SystmOne record, reviewing, amending or adding clinical codes in relation to vaccinations, newborn blood screening results, and uploading evidence onto the medical record.
- Where appropriate and as directed by the clinical team, accessing AccuRx to invite, book or communicate with patients in relation to current Proactive Care Campaigns.
- Following up any missing or flagging Safeguarding Concerns via appropriate channels.
- Ensure the effective delivery of Adhoc Proactive Care Campaigns such as Flu, by contacting patients via Accrux, managing responses and booking patients within a timely manner.
- Contacting and liaising with external organisations such as Health Visiting teams regarding Child Vaccinations.
WHAT WE’RE LOOKING FOR
- Strong understanding of NHS Primary Care and proactive care principles.
- Working knowledge of the Quality & Outcomes Framework (QOF), LES/DES, and the Investment & Impact Fund (IIF).
- Knowledge of clinical coding and data quality standards within primary care.
- Understanding of confidentiality, clinical governance, and information governance requirements.
- Experience working in Primary Care, HealthTech, or a similar healthcare operational environment.
- Hands‑on experience using SystmOne, AccuRx, or similar clinical IT systems.
- Experience running or supporting searches, audits, or population health tasks within clinical systems.
- Experience communicating with patients using SMS, email, telephone, or other digital tools.
- Experience working with multidisciplinary teams (clinical and non‑clinical).
DESIRABLE
- Training or certification in clinical coding, population health, or data quality.
- Understanding of GMS contract requirements and PCSE guidance (e.g., de‑registration processes).
- Knowledge of safeguarding processes and escalation pathways.
- Experience supporting proactive care campaigns such as Flu, Screening, LTC reviews, or Vaccination programmes.
- Experience managing shared NHS mailboxes or failsafe processes.
- Experience liaising with external services such as Health Visiting Teams, screening services, or immunisation providers.
- Experience working in a digital‑first or high‑volume primary care setting.
