About the Role
We are seeking two experienced Senior Care Partners to join the MannaCare Support at Home team on a permanent full-time basis.
One position will commence in October 2026 with the second position commencing in March 2027.
Reporting to the Home Care Team Lead, the Senior Care Partner will play a key role in supervising Care Partners and supporting older people to live safely and independently at home.
You will manage care coordination, develop individualised care plans, monitor participant needs and budgets, and support delivery across the Restorative Care and End-of-Life pathways, as well as Assistive Technology and Home Modifications where required.
Key Responsibilities
- Supervise, coach and support Care Partners
- Coordinate care and services for participants, ensuring their needs and goals are met
- Develop and review individualised care plans, budgets and service agreements
- Coordinate appropriate Restorative Care, End-of-Life and AT-HM supports
- Monitor participant needs, risks, service delivery and funding
- Ensure services comply with Support at Home requirements and aged care standards
- Conduct holistic assessments and maintain accurate care documentation
- Identify and escalate risks and ensure appropriate support and emergency plans are in place
- Participate in case conferences, continuous improvement activities and conducting staff appraisals where required.
Skills And Experiences
About You
- Demonstrated experience in care coordination, case management or a similar role within aged care, health or disability
- Understanding of the Support at Home Program and aged care standards
- Strong communication, empathy and cultural awareness
- Demonstrated experience mentoring and leading a team
- Strong organisational, problem-solving and time management skills
- Good numeracy and computer skills, including the ability to manage participant budgets and records
- Current Victorian Driver’s Licence
- Current NDIS Worker Screening Check
- Diploma-level qualification in Community Services, Aged Care, Case Management or equivalent experience is preferred
