About the role:
- As a Support At Home Care Partner, you will be responsible for promoting client participation and choice through the provision of effective, responsive, goal focused and outcomes-based support to Support At Home recipients.
- Working with clients from diverse backgrounds and their informal networks to ensure their individual needs are identified and supported.
- Effectively managing a caseload of clients, ensuring current client documentation and monthly targets are achieved.
- Working collaboratively within a multidisciplinary team to ensure outcomes are achieved in line with client goal directed care plans, and service objectives.
About you:
- Diploma of Community Services / Case Management or Allied Health.
- Experience with Case Management in particular managing a caseload of clients from goal directed care principles.
- Experience with budgeting and home care funds management.
- Strong knowledge of Aged Care Standards, the Aged Care Quality Code of Conduct, and the home care sector.
- Strong attention to detail.
- Excellent verbal and written communication skills.
- Demonstrated ability to work in a team and independently.
