We are seeking a dedicated and motivated Practice Nurse to join our friendly and supportive primary care team. This is an exciting opportunity to play a key role in delivering high-quality, patient-centred care within a welcoming and progressive practice environment. The successful candidate will provide a broad range of nursing services, including health promotion, chronic disease management, screening programmes and immunisations. Working closely with GPs and the wider multidisciplinary team, you will help ensure patients receive safe, effective and personalised care that supports their health and wellbeing.

Main duties of the job
As a Practice Nurse, you will provide a wide range of nursing services within our busy and friendly practice, caring for patients of all ages. Your role will include running nurse clinics, supporting patients with long-term conditions, carrying out health checks, vaccinations, and screening programmes, as well as promoting healthy lifestyles and wellbeing. You will work closely with GPs and other members of the practice team to provide high-quality, patient-centred care. Building positive relationships with patients will be an important part of the role, helping them to understand and manage their health with confidence.
You will be responsible for maintaining accurate clinical records, following practice policies and procedures, and helping to ensure safe and effective care is delivered at all times. You will also contribute to the day-to-day running of the practice and support your colleagues as part of a collaborative team. Job responsibilities
- Cervical cytology, including CSMS and smear safety monitoring
- Childhood immunisations
- Family planning support and advice
- Suture removal
- Simple and complex wound care and dressings
- Vaccination programmes including influenza, pneumococcal, shingles and pertussis
- Ordering and stock management of vaccines in line with cold chain protocols
- Infection control lead duties, including monthly audits
- Chronic disease management clinics
- Health promotion and preventative care activities
Person Specification
- UK registered nurse, significant post registration experience, experience in cervical cytology, knowledge of quality outcomes framework.
- Registered Nurse with current registration with the Nursing and Midwifery Council (NMC)
- Experience working in a primary care or community healthcare setting
- Competence in managing long-term conditions such as diabetes, asthma or COPD
- Experience delivering immunisations and vaccinations
- Knowledge of infection prevention and control and clinical governance
- Ability to assess, plan and deliver patient-centred care
- Strong communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Good organisational skills with the ability to prioritise workload effectively
- Competent IT skills and experience maintaining accurate clinical records
- Experience in cervical cytology (smear taking)
- Experience in wound care and dressings
- Knowledge of travel health and immunisations
- Experience using primary care clinical systems such as SystmOne
- Evidence of continued professional development or relevant specialist training
- Experience supporting health promotion and preventative care initiatives
- Understanding of local healthcare pathways and services
