Elm Lodge Surgery is seeking an Elderly Care Nurse to join our multidisciplinary general practice team in London. The role involves 6 sessions per week with flexible arrangements for the right candidate, including home visits to support frail older patients to live safely at home.
You will enjoy longer appointments (60 minutes+), case management for complex patients, and close collaboration with GPs and clinical pharmacists to proactively resolve issues before they escalate.
