An exciting opportunity has become available for a Senior Care Coordinator (Administrative) to join SAPA 5 Primary Care Network. This post is for 24 hours per week. The Admin Support Care Coordinator will provide comprehensive administrative support to ensure the effective and efficient running of the Primary Care Network (PCN). Working closely with the Digital and Transformation Lead, Clinical Directors, and member practices, the postholder will support the coordination of network activities and contribute to the smooth delivery of primary care services for the local population.
The role will focus on providing day-to-day administrative and organisational support across the PCN, including coordinating meetings, maintaining documentation, supporting project administration, and liaising with stakeholders. The postholder will assist with operational processes and provide administrative support to help deliver PCN priorities and initiatives, working collaboratively with community partners and the wider network team to support SAPA 5's vision and values.
Main duties of the job:- Provide day-to-day administrative support to the Digital and Transformation Lead across network activities.
- Support the smooth running of the network by assisting with routine operational tasks and escalating issues where appropriate.
- Run and distribute monthly reports to support IIF monitoring, income tracking, and project administration.
- Provide administrative support for the coordination and implementation of PCN projects and initiatives.
- Organise meetings, manage diaries, prepare agendas, and take and circulate accurate minutes.
- Attend meetings as required to provide administrative support and maintain effective communication with external partners.
- Assist with the preparation of business cases and funding applications by gathering and collating relevant information and data.
- Support the fair and transparent allocation of PCN-wide appointments and resources across member practices, using agreed criteria such as waiting list size, practice list size, and service demand data.

Other Responsibilities:- Maintain regular communication with core member practices, healthcare providers, local authorities, and other stakeholders to support effective working relationships.
- Provide administrative support to facilitate clear communication and information sharing between the PCN and its stakeholders.
- Liaise with external organisations providing staff to the PCN (e.g. SOAR, Sheffield Futures, Primary Care Sheffield and Sheffield Teaching Hospitals) to ensure timely communication and coordination.
- Provide administrative support to recruitment processes, including preparing documentation and coordinating induction packs, schedules, and training materials.
- Provide day-to-day administrative support to VCSE staff working within the network and assist in resolving or escalating operational queries as appropriate.
- Experience using Microsoft Office packages (Excel, Outlook, Teams, SharePoint).
- Previous experience working in an administrative role within healthcare, primary care, or the NHS.
- Experience supporting senior staff or managers with administrative tasks.
- Familiarity with NHS systems (e.g. EMIS, SystmOne).
- Experience with minute-taking and organising meetings.
- Experience coordinating diaries, appointments and events.
- Understanding of confidentiality and data protection in a healthcare environment.
- GCSE Grade A to C in English and Maths.
