Working as part of the Care Home Enhance Rapid Response service (CHERRS)
The aim of the service is to target urgent, emergency,unscheduled and acute episodes of care in patients residing in care homes whowould otherwise be at risk of hospital attendance or admission. The serviceprovides a rapid response which ensures the unwell patient receives promptexamination and treatment by an advanced practitioner You will be expected toassess episodes of acute/chronic illness and will be competent in the clinicalexamination, assessment, diagnosis, and treatment of the patient, includingprescribing where appropriate.
Main duties of the job
- Must have Prescribing Qualification
To succeed, you will be a practicing advanced nursepractitioner/advanced paramedic with previous experience in Primary or Community Care. You will demonstrate a high standard of clinical skills. Youwill also have the ability to communicate, liaise and interact effectively withmulti professional teams across organisational boundaries, including theambulance service, primary and community care, social services and secondary care providers.
Strong organisation and time management skills areessential, as is the ability to prioritise your workload. You will also haveexperience using various clinical IT systems, in addition to basic IT literacy.
About us
The ELM Alliance is a GP Federation of all independent GPpractices throughout South Tees which covers over 300,000 patients. Members ofthe Alliance remain independent organisations, whilst collaborating in thedevelopment of local primary care.
The ELM Alliance CHERRS service was first commissioned inJune 2017. The service is provided Monday to Sunday 8am – 6pm, including BankHolidays.
Job responsibilities
KEY RESPONSIBILITIES
Clinical
To provide assessment at first point of contact by attendingto patients in care/nursing homes presenting with acute/chronic conditions
To prioritise patients including urgent situations,demonstrating the use of clinical assessment skills to obtain history, patientexamination, diagnosis and treatment, including non medical prescribing whereappropriate
To evaluate clinical information from examination andhistory taking, and initiate appropriate treatment and/or referral
To undertake and interpret diagnostics and take appropriateaction to manage patient care.
To signpost appropriately to other services in theidentified healthcare pathway
To refer patients to appropriate care settings, withinlocally agreed healthcare pathways
To discharge or treat, as appropriate and within the scopeof practice
To prescribe as an independent non-medical prescriber
To maintain accurate and systematic documentation of patientinteractions and clinical care provided, including adverse events
To provide clinical advice to other members of the teamwhere appropriate
Confidentiality:
Under the Data Protection Act 2018, the clinician must maintainthe confidentiality of information about patients and staff. Sensitiveinformation in relation to a persons health and circumstances is of aconfidential nature and information gained must not be communicated to otherpersons except in the recognised course of duty. Unauthorised disclosure ofconfidential information (including information about the organisationsbusiness) will result in disciplinary action and may lead to your dismissal. Ifat any time you are in doubt regarding the use of information in the course ofyour duties, you should seek advice from the designated line manager beforecommunicating any such information to any third party.
Data Protection:
The clinician will not use data held by ELM ALLIANCE or providedata to a third party for any purpose other than required within the remit oftheir duties.

Maintains consistent high professional standards and acts inaccordance with the relevant code of professional conduct.
The advanced practitioner will be required to attend allmandatory training expected to participate in the performance review process.

Qualifications
- Current registered Nurse (NMC) or registered Paramedic (HPC)
- Evidence of continued professional development
- University accredited advanced physical assessment skills course level 6/7 / ECP qualification Non-Medical Prescribing qualification
- Attended X-ray/IRMER course Teaching and assessing qualification
