The predicted salary is between 13 - 16 £ per hour.

We are looking for Care Navigator(s) to join our experienced team to support the practice clinical team by signposting patients to the appropriate healthcare professional or service, working as part of the practice multidisciplinary team. The care navigator will also be expected to undertake reception duties as part of the role, supporting the administrative team in delivering a polite and professional service to the entitled patient population.

We require 2 x part-time Care Navigators (16.5hrs & 22hrs) to work Monday, Tuesday, Thursday & Friday. The shifts will be either 08:00-13:30 or 13:15-18:45. These shifts are non-negotiable.

Responsibilities:

  • Process and signpost patients
  • Initiate contact with and respond to requests from patients, team members and external agencies
  • Enter read-code data on Emis
  • Managing registrations
  • Input data into the patients healthcare records
  • Direct requests for information to the appropriate team
  • Manage queries as necessary in an efficient and polite manner
  • Carry out system searches as requested
  • Maintain a clean, tidy, effective working area
  • Monitor and maintain the reception area and noticeboards
  • Support clinical staff with general tasks as needed
  • Participate in practice audit as directed
  • Action incoming emails
  • Complete opening and closing procedures in accordance with the duty rota
  • Support pharmacist(s) in the management of prescriptions
  • Manage the movement of patient medical records
  • Attend small group and team meetings
  • Monitor and replenish supplies
  • Medical Record Envelope management
  • Support the surgery team to aid the smooth running of the business

  • Skills:
  • Excellent communication skills (written and oral)
  • Clear, polite telephone manner
  • Effective time management (planning & organising)
  • Ability to work as a team member and autonomously
  • Good interpersonal skills
  • Ability to follow clinical policy and procedure
  • Competent in the use of Office and Outlook
  • Qualifications:
  • Educated to GCSE level or equivalent
  • Active signposting or Care Navigator training qualification
  • Healthcare qualification (level 2) or working towards gaining equivalent level
  • Experience:
  • Experience of working with the general public
  • Experience of working in a healthcare setting
  • Experience of working in a primary care environment
