You will work closely with the Clinical Lead and will support the ongoing development and smooth operation of the care package, helping to ensure that care remains responsive, well-coordinated, and bespoke to the client’s evolving needs. Role requirements:- Coordinate the daily running of the client’s care package, ensuring all operational elements are effectively managed and followed through.
- Act as a key point of contact for the client, their family, and the wider care team, supporting clear and professional communication.
- Support the Clinical Lead by coordinating actions from reviews, meetings, incidents, and changes in the client’s condition or care needs.
