Responsibilities: Provide side-by-side interventions focusing on problem solving, skills training, modeling behavior, optimism, and encouragement Deliver interventions in the office, virtually, in the community, and in clients\’ residences based on client preference and clinical direction Enroll eligible individuals in care management, including BHCP and Health Home Collaborate with Care Transitions Team to triage hospital admissions data and develop follow-up strategies during inpatient stays Assist in ongoing assessment of symptoms and responses to interventions Participate in developing culturally competent, strength-based, realistic, and recovery-promoting treatment plans Ensure active involvement of individuals in their treatment planning, respecting their preferences Document progress in a measurable, objective, and clinical manner Coordinate timely face-to-face contact with enrollees following inpatient discharge, adhering to MassHealth requirements Qualifications: Bachelor’s or Master’s degree in psychology, social work, or related field with at least three years of experience Strong commitment to recovery-oriented services Ability to establish supportive relationships with individuals with severe mental illnesses, children, and families, respecting their treatment preferences Effective communication skills with individuals in rehabilitation and treatment Salary: $53,000 annually, including a 6% bilingual differential.
