Our ideal candidate will have the skills to establish supportive relationships, excellent communication, crisis management, and problem-solving skills.
Responsibilities
- Provide education and consultation to clients and staff on medical health, medications, and general medical topics
- Deliver one-on-one training tailored to specific individual needs
- Develop and lead groups and trainings for clients and staff on general health and wellness topics, including nutrition, exercise, and smoking cessation
- Assist clients in accessing community resources that support their health and wellness goals
- Meet with clients and staff to discuss medical diagnoses, causes, symptoms, complications, and available treatment options
- Perform direct medical care for a person as appropriate and ordered by a physician, which may include monitoring of vitals, wound care, etc.
- Review medical documentation for each assigned client and provide consultation and coordination of care as needed
- Coordinate overall medical care for clients and ensure necessary follow-up is completed
- Provide health and wellness interventions focused on education and skill-building
- Support the team and clients by introducing tools for ongoing symptom assessment, evaluating treatment effectiveness, and identifying appropriate responses or next steps

- License LPN/RN
- Experience working with adults with severe mental illness
Schedule: Monday-Friday, 8am-4pm
Salary: $60,000 - $60,000 USD
