The Role

As Assistant Care Manager, you will work closely with the Care Manager to oversee daily service delivery, lead a team of Care Professionals, and uphold the high standards that define this organisation. You will be hands-on in the field, present for your team, and invested in the outcomes of every client. The client will also consider experienced Senior Care Coordinators who are ready to take the next step.

Key Responsibilities

  • Lead, mentor, and develop a team of Care Professionals
  • Conduct home visits, care assessments, and quality assurance checks
  • Introduce Care Professionals to clients and build strong, lasting relationships
  • Write, review, and update personalised care plans
  • Monitor performance and drive continuous improvement
  • Maintain accurate, compliant care records in line with CQC standards
  • Collaborate with families, GPs, and wider healthcare professionals
  • Handle concerns with empathy, professionalism, and a solutions-focused approach
  • Participate in the on-call rota and support operational management
