About the Role

  • Part-time (0.6 FTE) - Mon, Tues, and Fri (negotiable)
  • Located in Eastwood with travel to Southern Eldercare sites required

Eldercare is seeking a Social Worker to join an established Social Work service across our large, multi-site aged care organisation. Eldercare's model of care recognises the broader psychosocial context of our residents' lives, and our Social Workers offer expertise around decision-making capacity, resident risk, family dynamics and other factors affecting resident welfare.

Reporting into a Senior Social Worker, and working closely with the multi-disciplinary team, you will improve health and wellbeing outcomes of residents through:

  • Collaborative management of significant and complex resident and family relationships
  • Providing guidance and advocacy through periods of grief, loss, and adjustment, including admission, palliative care, and end-of-life decisions.
  • Assisting residents and families with South Australian Civil and Administrative Tribunal (SACAT) applications, including guardianship and administration orders.
  • Psychosocial, risk, and capacity assessments
  • Individual and family work
  • Navigating relevant external health and welfare services
  • Care planning and advocacy for individual residents

What You Will Bring:

  • Tertiary qualifications in Social Work
  • Eligibility to attain Australian Association of Social Work (AASW) membership
  • At least 3 years' experience working with older people and their families, ideally in aged care, acute health, or community services.
  • Knowledge of decision-making capacity and restrictive practices in aged care
  • Excellent verbal and written communication skills, with the ability to build effective professional relationships with stakeholders
  • A valid South Australian Driver's Licence
