The role involves working with the Programme Manager on the design, delivery, and evaluation of targeted initiatives that promote physical, mental, and social wellbeing across Pembrokeshire. This role is pivotal to the Economic Inactivity Trailblazer, focusing on developing one-to-one and group-based programmes designed to improve individuals' ability to manage their health and reduce its impact on their capacity to work. You will provide strength-based support, organising and ensuring the delivery of structured interventions, identifying the needs, aspirations, and challenges of residents to foster sustainable improvements.
Key responsibilities include:
- Design & Deliver: Lead the development, coordination, and delivery of tailored community programmes, workshops, and wellbeing events, ensuring activities are inclusive, outcome-focused, and responsive to local need.
- Supervise & Support: Provide day-to-day supervision, guidance, and project guidance to a small team of Pathways Advisers, including supporting the setting of clear objectives and offering regular one-to-one support.
- Collaborate: Build and maintain strong working relationships with a wide range of stakeholders, including GPs, healthcare professionals, and local organisations.
- Evaluate: Oversee the monitoring and evaluation of programme impact, ensuring accurate data collection and meaningful use of both qualitative and quantitative feedback.
- Champion: Act as an ambassador for the Trailblazer project across Pembrokeshire communities, promoting awareness of integrated services.

Requirements:
- A Level 4 qualification or higher in a health, social care, or rehabilitative field (or equivalent experience).
- Demonstrable experience in staff supervision, including performance guidance and team development.
- Strong analytical and reporting skills, with the ability to interpret and present both qualitative and quantitative data.
- A full UK driving licence and access to a vehicle, with the ability to travel across dispersed rural communities.
