Role
To provide person-centered support that enables people with a range of needs to work towards and achieve their own personal goals and aspirations. You will be working out of the EDGE Ahead Hub, supporting and encouraging clients to engage in a variety of activities on a 1:1 basis as well as within small workshop groups and out in the local community
Duties and Responsibilities
Understanding of Needs: Developing strong professional relationships with clients aged 11yrs + who may have learning disabilities, Neurodiversity or social emotional mental health needs.
Following the guidance in each person’s individual action plan and risk assessment, support them to manage their anxiety and behaviour in a positive way and to keep them and others safe
Planning and Delivery
- Helping to set up and prepare for workshops and activities within the hub daily, as well as clear away and clean at the end of the day
- Supporting clients on community outings and work experience, these may include activities such as mountain biking, walking, equestrian therapy etc
- Working with other professionals involved with the client (SEND, SW, teachers/tutors, employers/line manager)
Assessment and Paperwork
- Keeping monitoring records using photographs/videos and written reports
- Completing the relevant sections on Log My Care
- Communicating frequently and effectively with colleagues and families about the needs of the clients
