Overview
Eden Futures delivers support services for adults aged 18 and over who have learning disabilities, autism, mental ill health and associated complex needs.
We are recruiting Support Workers to join the team across our Supported Living services, supporting individuals in their own homes as well as in the community, helping them to live as independently as possible.
Responsibilities
- Provide person-centered support to individuals in their own homes and in the community, promoting independence and wellbeing.
- Assist with daily living tasks (personal care, daily routines, medication prompting where appropriate) while maintaining dignity and respect.
- Support participation in community activities, appointments, and social engagement.
- Monitor and report changes in health, wellbeing, or safety to the care team, and document relevant information accurately.
- Follow care plans, policies, and safeguarding procedures; maintain confidentiality and professional boundaries.
Qualifications and Requirements
