Are you a dedicated and experienced Social Worker looking to make a meaningful impact? Nottinghamshire County Council is offering a unique opportunity to join our highly specialized Transforming Lives Team or Community Forensic Team, supporting adults with complex needs, promoting independence, and safeguarding well-being.

About the Teams:

Transforming Lives Team:

Undertake Care Act assessments, support planning, risk assessments, and discharge planning for individuals under the Learning Disability and Autism Programme with enduring mental health needs.

Work creatively to support adults with significant care needs, applying a strengths-based approach that promotes independence and well-being.

Engage in Court of Protection work, including managing community Deprivation of Liberty (DoL) cases.

Community Forensic Team:

Deliver Care Act assessments, support planning, and manage discharges for forensic patients transitioning to the community, often with high-risk profiles.

Act as a Social Supervisor, providing detailed reports to the Ministry of Justice for individuals subject to Section 41 of the Mental Health Act.

Balance risk management with the promotion of independence and community integration., Conduct thorough assessments and develop individualized support plans in line with the Care Act 2014.

Collaborate with multi-disciplinary teams to ensure holistic and effective support for service users.

Apply creative problem-solving to meet complex care needs and overcome challenges.

Maintain compliance with all relevant legal frameworks, including the Mental Health Act, Care Act, and Court of Protection requirements.

Promote independence and well-being while ensuring safety and safeguarding for all individuals.

Qualified Social Worker with current registration with Social Work England.

Experience in working with adults with learning disabilities, autism, enduring mental health needs, and/or forensic profiles.

Knowledge of Court of Protection and community Deprivation of Liberty (DoL) processes.

Familiarity with the Mental Health Act, particularly Section 41 and Social Supervision responsibilities.

Ability to work effectively in high-pressure environments, balancing risk management with person-centered support.

Full driving license and access to a vehicle for work purposes.

Competitive pay rate of £34 per hour.
