The Role

Eden Brown Synergy is currently recruiting for an experienced Occupational Therapist to join the Adult First Contact Service (AFCS) within the London Borough of Sutton. This is an exciting opportunity for an OT who is passionate about promoting independence, prevention and strength-based practice. Working at the front door of Adult Social Care, you will play a key role in screening, triaging and prioritising referrals and ensuring residents receive the most appropriate support at the earliest opportunity. You will work as part of a multidisciplinary duty team alongside Social Workers, health partners and other professionals to achieve positive outcomes for residents.

Key Responsibilities

  • Screen and prioritise complex referrals received through the Contact Centre.
  • Use professional judgement to determine the most appropriate pathway for residents.
  • Complete person-centred, holistic and proportionate assessments.
  • Apply strength-based and preventative approaches to promote independence and wellbeing.
  • Provide professional guidance and quality assurance to Enabling Independence Officers and Trusted Assessors.
  • Assess and recommend appropriate equipment, minor adaptations and major adaptations.
  • Demonstrate equipment and ensure residents, families and carers understand its safe and appropriate use.
  • Assess complex cases involving major adaptations, structural alterations, extensions, through-floor lifts and specialist equipment.
  • Support residents where rehousing may be required to maintain independence.
  • Work collaboratively with Housing, Social Care, Environmental Health, health services and other partner agencies.
  • Prepare accurate briefs and sketch diagrams for building surveyors where required.
  • Ensure adaptations procedures are implemented appropriately and in line with relevant policies and legislation.
  • Escalate complex cases and provide professional advice to colleagues and partner services.

Requirements

  • A Degree or equivalent qualification in Occupational Therapy.
  • Current HCPC registration.
  • Significant experience working as an Occupational Therapist.
  • Ideally, previous experience within Local Authority, Adult Social Care or community settings.
  • Experience of screening, triaging and completing holistic assessments.
  • Strong knowledge of equipment, adaptations and moving and handling.
  • Knowledge of relevant legislation, including the Care Act and Mental Capacity Act.
  • Understanding of safeguarding within an adult social care environment.
  • A strong commitment to strength-based, preventative and person-centred practice.
  • The ability to work autonomously while contributing effectively to a multidisciplinary team.
