This is an exciting opportunity for an OT who is passionate about promoting independence, prevention and strength-based practice. Working at the front door of Adult Social Care, you will play a key role in screening, triaging and prioritising referrals and ensuring residents receive the most appropriate support at the earliest opportunity. You will work as part of a multidisciplinary duty team alongside Social Workers, health partners and other professionals to achieve positive outcomes for residents. Key Responsibilities Screen and prioritise complex referrals received through the Contact Centre. Use professional judgement to determine the most appropriate pathway for residents.
