Eden Brown Synergy are currently looking for an experienced Occupational Therapist to join the Haverhill Area Integrated Team within Adult Social Care in Suffolk. This is an excellent opportunity to work within a supportive and flexible team environment, helping adults with care and support needs to remain independent within their own homes.
Key Responsibilities:
- Manage a varied and increasingly complex Occupational Therapy caseload with support and supervision.
- Undertake face-to-face, asset-based assessments for adults and carers.
- Assess functional needs and identify appropriate solutions including:
- Reablement potential
- Equipment provision
- Adaptations
- Digital solutions
- Respond to urgent moving and handling requests.
- Develop, implement and review reablement plans.
- Set goals and outcomes with adults and carers.
- Recommend and specify major and minor adaptations.
- Complete referrals and applications to:
- Community Equipment Services (CES)
- Registered Social Landlords
- Orbit Home Improvement Agency
- Borough and District Councils
- Assess for specialist equipment alongside suppliers, ensuring best-value solutions.
- Work collaboratively with health professionals and partner agencies.
- Maintain accurate and confidential records on relevant databases.
- Apply Care Act Eligibility criteria and relevant legislation in practice.
- Support urgent and standard team workloads.
- Participate in supervision, meetings and ongoing professional development.
- Provide Occupational Therapy advice and guidance to colleagues and Independence & Wellbeing Practitioners (IWPs).
- Support and supervise a small number of IWPs where agreed.
Essential Requirements:
- Qualified Occupational Therapist with relevant registration.
- Experience within Adult Social Care.
- Strong assessment and case management skills.
- Ability to work independently and as part of a multidisciplinary team.
- Driving licence and access to a vehicle is essential.
