Eden Brown Synergy is currently recruiting for a Qualified Social Worker (Permanent) to join the Ageing Well Adults Community Team within Southend-on-Sea Borough Council. This is an excellent opportunity for an experienced and motivated Social Worker to join a forward‑thinking service focused on promoting independence, wellbeing, and strengths‑based practice within adult social care.
About the Role
You will be working within the Ageing Well Service, supporting older adults and adults with mental ill health across Southend-on‑Sea (including out‑of‑borough cases). The role focuses on promoting independence, reducing reliance on long‑term care, and enabling individuals to remain within their communities wherever possible.
Responsibilities
- Undertake strengths‑based assessments and care planning
- Promote independence, resilience, and community‑based support
- Work in partnership with health colleagues, GPs, hospitals, and community organisations
- Carry out safeguarding enquiries, Mental Capacity Act assessments, and (where required) DoLS work
- Support individuals to achieve personalised outcomes using a person‑centred approach
- Contribute to innovative and collaborative multi‑agency working
Salary & Benefits
- Salary: £39,152 – £45,091
- Hybrid working (minimum 2 days on site)
