Eden Brown Synergy are currently recruiting for Wiltshire Council who are looking for an experienced Advanced Occupational Therapist to join the Reablement Team based in Trowbridge.
The role will focus primarily on hospital discharge work, undertaking assessments, determining care packages and support plans, and managing a large and complex caseload to maintain or improve client wellbeing in line with service procedures and national legislation.
