We are looking for an experienced and motivated Qualified Social Worker (QSW) to join Rochdale Adult Care\βs Hospital Discharge Team. Based in Middleton, you will cover all hospital discharges across various sites, including out-of-borough hospitals.

You\βll be working as part of our Home First Hospital Discharge Hub, supporting Rochdale residents to return home or move to the most suitable care setting safely and promptly. This is a crucial role where you\βll help people regain independence and ensure the right support is in place at the right time.
Key Responsibilities
Undertake statutory Social Work duties in line with the Care Act (2014) and Mental Capacity Act (2005)
Complete strength-based assessments and develop care plans to support safe and timely hospital discharges
Work collaboratively with health professionals, reablement teams, and community partners
Support individuals to access the right care pathways, including Discharge to Assess (D2A) and Continuing Healthcare (CHC)
Maintain accurate and detailed case records using both manual and electronic systems
Contribute to the transformation journey of Rochdale Adult Care, supporting continuous improvement in outcomes for residents
About You
To be successful in this role, you\βll need:
A Social Work qualification (Degree, DipSW, CQSW) and current Social Work England registration
Minimum of 2 years\β experience working in a hospital or hospital discharge team
Excellent organisational and communication skills
The ability to manage competing priorities in a fast-paced environment
Strong ICT skills (Word, Excel, Outlook, case recording systems)
A valid driving licence and access to a vehicle (essential)
A proactive, compassionate approach to supporting adults and their families
