We are supporting a client in the London area with a S117 Joint Funding project that requires additional resource. This is a hybrid role although you will need to be available for face to face visits.

Day to Day Duties:

  • Deliver care plans and interventions.
  • Collaborate with multi-disciplinary teams, including social workers and healthcare professionals.
  • Monitor and assess client progress, ensuring their health and well-being.
  • Provide support with activities of daily living, promoting independence and choice.
  • Conduct risk assessments and manage care-related documentation.
