Join Redbridge's Integrated Discharge to Assess Team. Redbridge is seeking an experienced and motivated Occupational Therapist to join our locality-based Discharge to Assess (D2A) team. This is an excellent opportunity to work within a supportive, multi-disciplinary environment, helping individuals safely transition from hospital and maximise their independence within the community.

Working as part of our integrated Community Health and Adult Social Care service, you will carry a manageable caseload of approximately four cases per week, consisting of both standard and highly complex occupational therapy assessments.

About the Team

Our Discharge to Assess team is a locality-based, multi-disciplinary service committed to delivering person-centred, strengths-based interventions. You will collaborate closely with health and social care professionals to assess needs, recommend appropriate equipment and adaptations, and support timely, safe discharges from hospital.

Key Responsibilities

  • Complete comprehensive occupational therapy assessments for adults with a range of health conditions.
  • Undertake holistic, person-centred assessments and develop effective support plans.
  • Assess for, recommend and review simple and complex equipment and major adaptations.
  • Provide expert advice on manual handling and safe moving and handling practices.
  • Work collaboratively within a multi-disciplinary team to facilitate safe hospital discharges.
  • Manage a varied caseload, prioritising and progressing complex cases independently.
  • Deliver reviews and monitor the effectiveness of interventions and equipment provided.
  • Ensure practice complies with relevant legislation, policies and professional standards.

Essential Requirements

  • Excellent knowledge, skills and experience in assessing for and recommending complex equipment and manual handling solutions.
  • Experience working within a multi-disciplinary team, undertaking complex occupational therapy, holistic person-centred assessments, support planning and reviews for adults with a range of health conditions and their carers.
  • A strong understanding of the legal framework for Adult Social Care and relevant housing legislation.
  • Proven ability to manage, prioritise and progress a complex caseload independently.
  • Extensive experience assessing for, recommending and reviewing both simple and complex equipment, including teaching safe use in accordance with relevant legislation, policies and safe working practices.

Essential Experience

  • Extensive knowledge of specialist equipment, major adaptations and manual handling.
  • Experience working autonomously with excellent workload management and prioritisation skills.
  • Previous experience working within a multi-disciplinary setting.
  • Ability to undertake complex occupational therapy assessments and deliver effective interventions.

Essential Qualifications

  • Professional Occupational Therapy qualification recognised by the Health and Care Professions Council (HCPC).
  • Current HCPC registration.
