Are you a Transitions Social Worker looking for a new role ? Hackney council are looking for an experienced Social Worker to join their team. The Integrated Learning Disabilities Service (ILDS) is a multi-disciplinary service comprised of social workers, psychologists, psychiatrists, nurses, occupational therapist, physiotherapist, dietician, speech and language therapists, and business support staff. The service completes Care Act assessments and reviews of need, functional and other specialist assessments. It also provides social work and health support and interventions for adults who have a confirmed learning disability, and support for their carers. The service is fully integrated and each of the teams is multidisciplinary. There is a real opportunity to improve the outcomes and experiences for (young )adults with a learning disability working with a range of professionals, the residents themselves and their families.
