Eden Brown Synergy is currently recruiting an experienced Qualified Social Worker (QSW) to join the Adult Social Care Review Team in Gloucestershire.
This is an excellent opportunity for a registered Social Worker with significant Adult Social Care experience to undertake focused social care reviews, particularly within the Supported Accommodation Service.
You will work collaboratively as part of a multidisciplinary team (MDT), reviewing existing care and support arrangements to ensure individuals receive appropriate support that meets their needs and promotes independence.
Location: Gloucestershire
Pay Rate: £35 per hour
Contract: Temporary - Initial 3 Months (Possible Extension)
Weekly Allowance: £150 Accommodation / Commuter Mileage Allowance
Key Responsibilities
- Undertake focused and timely social care reviews for individuals within the Supported Accommodation Service.
- Review existing care and support arrangements to ensure they remain appropriate and outcome-focused.
- Work collaboratively as part of a multidisciplinary team (MDT).
- Engage with service users, families, carers and relevant professionals to understand individual needs and circumstances.
- Identify changes in care and support requirements and recommend appropriate interventions.
- Apply professional judgement when reviewing care packages and making recommendations.
- Ensure reviews are person-centred and promote independence, choice and positive outcomes.
- Maintain accurate and up-to-date case records and documentation.
- Communicate effectively with partner agencies and relevant stakeholders.
- Demonstrate flexibility and adaptability in responding to service requirements.
Essential Requirements
- Qualified Social Worker with a recognised Social Work qualification.
- Current registration with Social Work England.
- Significant experience within Adult Social Care.
- Experience undertaking social care assessments and reviews.
- Ability to work effectively within a multidisciplinary team.
- Excellent communication, assessment and report-writing skills.
- Flexible and adaptable approach to work.
- Ability to work independently and manage competing priorities.
- Willingness to work agilely, with flexible on-site attendance as required.
