Eden Brown Synergy are currently looking for an experienced Qualified Social Worker to join the Mental Health Team at Bromley Council.

Duties and Responsibilities

  • Manage an allocated caseload of adults experiencing mental health needs.
  • Undertake comprehensive assessments, reviews, and care and support planning in line with the Care Act 2014.
  • Assess risk and identify appropriate interventions and support to promote independence and wellbeing.
  • Undertake Mental Capacity Assessments and Best Interests decision-making where required.
  • Respond appropriately to safeguarding concerns and work in accordance with statutory requirements.
  • Work collaboratively with service users, carers, health professionals, housing, and other partner agencies.
  • Develop person-centred, outcome-focused support plans based on individual needs and strengths.
  • Maintain accurate case records, assessments, reviews, and reports.
  • Work effectively within a multi-disciplinary environment to achieve positive outcomes for service users.

Essential Requirements

  • Social Work England (SWE) registration.
  • Social Work qualification.
  • Previous experience relevant to Adult Social Care / Mental Health Social Work.
  • Knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding procedures.
  • Experience undertaking assessments, reviews, risk assessments, and care planning.
  • Strong communication and multi-agency working skills.
