Eden Brown Synergy are currently looking for an experienced Qualified Social Worker to join the Mental Health Team at Bromley Council.
Duties and Responsibilities
- Manage an allocated caseload of adults experiencing mental health needs.
- Undertake comprehensive assessments, reviews, and care and support planning in line with the Care Act 2014.
- Assess risk and identify appropriate interventions and support to promote independence and wellbeing.
- Undertake Mental Capacity Assessments and Best Interests decision-making where required.
- Respond appropriately to safeguarding concerns and work in accordance with statutory requirements.
- Work collaboratively with service users, carers, health professionals, housing, and other partner agencies.
- Develop person-centred, outcome-focused support plans based on individual needs and strengths.
- Maintain accurate case records, assessments, reviews, and reports.
- Work effectively within a multi-disciplinary environment to achieve positive outcomes for service users.

Essential Requirements
- Social Work England (SWE) registration.
- Social Work qualification.
- Previous experience relevant to Adult Social Care / Mental Health Social Work.
- Knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding procedures.
- Experience undertaking assessments, reviews, risk assessments, and care planning.
- Strong communication and multi-agency working skills.
