We are currently recruiting for two experienced Adult Social Workers to join Norfolk County Council hospital discharge services, based at:

  • Norfolk and Norwich University Hospital (NNUH), NR4 7UY - Acute Hospital Discharge Team
  • Willow Therapy Unit, Norwich Community Hospital, NR2 3TU - Community Hospital / Rehab Discharge Team

Both roles are available for an immediate start on a 6-month contract. These are exciting opportunities to work within fast-paced multidisciplinary hospital environments, supporting safe and timely discharge through a Home First approach, promoting independence, recovery, and person-centred care.

Key Responsibilities:

  • Undertake Care Act 2014 assessments and support planning
  • Complete Mental Capacity Assessments and Best Interest decisions
  • Support safe and timely hospital discharge planning
  • Work closely with health professionals, MDTs, and families
  • Undertake Continuing Healthcare (CHC) reviews
  • Promote independence and reduce delayed discharges

Requirements:

  • Qualified Social Worker registered with Social Work England
  • Experience in hospital discharge or community adult social care
  • Strong knowledge of Care Act 2014, MCA 2005, and safeguarding adults
  • Ability to work in a fast-paced acute or community hospital setting
  • Excellent communication and MDT working skills

Rate: £44.65 per hour
