Working Pattern: Monday - Friday | 37.5 Hours Per Week
Pay Rate: Up to £31 per hour Umbrella equivalent
Role Overview:
We are currently recruiting for experienced Adult Social Workers to support a busy Adult Social Care service within a community NHS and Local Authority integrated setting. The service is experiencing high demand and increasing waiting lists, therefore requires experienced practitioners who can hit the ground running within statutory Adult Social Care pathways.
Key Responsibilities:
- Complete Section 9 Adult Social Care Assessments and Support Plans
- Undertake Mental Capacity Assessments in line with the Mental Capacity Act 2005
- Complete Section 42 Safeguarding Enquiries
- Carry out risk assessments and risk management planning
- Complete Carers Assessments
- Support hospital discharge planning and commissioned care arrangements
- Manage complex cases within a multidisciplinary environment
- Maintain accurate documentation and case recording using electronic care management systems
- Liaise with families, health professionals, providers, and partner agencies
Essential Requirements:
- Qualified Social Worker with Social Work England registration
- Strong understanding of the Care Act 2014 and Mental Capacity Act 2005
- Previous Adult Social Care experience
- Experience completing safeguarding enquiries and statutory assessments
- Ability to work independently and manage a varied caseload
- Good communication, report writing, and MDT working skills
