Eden Brown Synergy is currently recruiting for Wiltshire Council for the position of Advanced Occupational Therapist with the Reablement Team in Trowbridge, Wiltshire. The role focuses primarily on hospital discharge work, including assessments, determining care packages and support plans, and managing a large and complex caseload to maintain or improve client wellbeing in line with service procedures and national legislation.
