An excellent opportunity has arisen for an Advanced Occupational Therapist to join the Redbridge Intermediate Care Team. This role involves working with stroke patients in the community, providing a high level of rehabilitation and support. You will play a key part in the multidisciplinary team (MDT), demonstrating strong knowledge of new stroke rehabilitation guidelines and contributing to patient-centred goal achievement. The role includes carrying a complex caseload, conducting assessments, and developing rehabilitation plans.
- Provide community rehabilitation for stroke patients with new diagnoses, focusing on functional independence
- Conduct detailed assessments and treatment for patients in their home environment
- Work closely with multidisciplinary teams (MDTs) to achieve patient goals
- Demonstrate knowledge of stroke rehabilitation guidelines and apply them to manage complex patient needs
- Assist in ordering equipment to optimize patient independence and improve quality of life
- Ensure patient-centred care, actively involving patients in decision-making
- Provide education and support to families and carers
Essential Requirements
- Degree in Occupational Therapy
- HCPC registration
- Experience in community rehabilitation (NHS experience preferred)
- Experience working with stroke patients in the community setting
- Strong assessment and report writing skills
- Excellent communication skills to work within MDTs and with patients and families
Additional Information
- 37.5 hours per week (Mon-Fri, 9am-5pm)
- Role requires travel across Redbridge borough
- Driving licence required
