Practice Nurse - Hammersmith & Charing Cross (Central London)

Start Date: ASAP

End Date: Ongoing

Location: Hammersmith & Charing Cross, Central London

Pay Rate: Negotiable (Band 6 equivalent - £25 - £35 per hour)

Booking Special Terms: All candidates must hold active NMC registration, an enhanced DBS (on the update service), and two professional references prior to submission.

Job Summary: Eden Brown Synergy is currently supporting a leading NHS organisation in Central London seeking two experienced Practice Nurses to join their Primary Care Team across Hammersmith and Charing Cross GP sites.

Duties and Responsibilities:

  • Deliver high-quality nursing care within a primary care setting
  • Conduct chronic disease and long-term condition reviews (e.g. diabetes, asthma, COPD, hypertension)
  • Administer immunisations and vaccinations for all age groups
  • Perform cervical screening and support women's health clinics
  • Participate in clinical audits, infection control, and health promotion initiatives
  • Maintain accurate patient records using SystmOne (experience desirable)
  • Work collaboratively with GPs, Clinical Pharmacists, and the wider MDT

Requirements:

  • Registered Nurse (NMC PIN) with Practice Nurse qualification
  • Experience in a GP or primary care environment
  • Confident in chronic disease management and vaccination protocols
  • Cervical smear training desirable
  • Strong communication and documentation skills
