Eden Brown Synergy is currently recruiting for an experienced Qualified Social Worker to join the Home First - Hospital Discharge team in Leeds. This is an excellent opportunity to work within a fast-paced service supporting timely and safe hospital discharges.

- 37 hours per week
- Hybrid working (minimum 3 days office-based)
