Eden Brown Synergy is seeking an experienced Qualified Social Worker to join hospital social work teams in Fylde & Wyre. This role involves supporting hospital discharge pathways, triaging referrals, and ensuring timely social care interventions for adults.
Key Responsibilities:
- Triage referrals through the Single Point of Access (SPA) and support hospital discharge processes
- Apply eligibility criteria under the Care Act 2014
- Complete Continuing Health Care (CHC) assessments and reviews
- Conduct Mental Capacity Act (MCA) 2005 assessments
- Work closely with multidisciplinary teams to ensure smooth discharge and continuity of care
Essential Requirements:
- Qualified Social Worker (SWE registered)
- Access to a vehicle and ability to drive (travel required)
- Previous experience in hospital discharge pathways and adult social care assessments
- Strong knowledge of Care Act eligibility criteria and MCA assessments
- Ability to work independently and manage a varied caseload
