Qualified Social Worker - Hospital (Lancashire, Fylde & Wyre)

Rate: £33 per hour

Location: Fylde & Wyre, Lancashire

Eden Brown Synergy is seeking an experienced Qualified Social Worker to join hospital social work teams in Fylde & Wyre. This role involves supporting hospital discharge pathways, triaging referrals, and ensuring timely social care interventions for adults.

Key Responsibilities:

  • Triage referrals through the Single Point of Access (SPA) and support hospital discharge processes
  • Apply eligibility criteria under the Care Act 2014
  • Complete Continuing Health Care (CHC) assessments and reviews
  • Conduct Mental Capacity Act (MCA) 2005 assessments
  • Work closely with multidisciplinary teams to ensure smooth discharge and continuity of care

Essential Requirements:

  • Qualified Social Worker (SWE registered)
  • Access to a vehicle and ability to drive (travel required)
  • Previous experience in hospital discharge pathways and adult social care assessments
  • Strong knowledge of Care Act eligibility criteria and MCA assessments
  • Ability to work independently and manage a varied caseload
