Eden Brown Synergy is currently recruiting for an experienced Transitions Social Worker to join a team in Kirklees. We are seeking a qualified and experienced Social Worker with a strong background in Learning Disabilities (LD) and transitions social work. The successful candidate will support young people transitioning from children's services into adult social care, ensuring they receive appropriate support and services to promote independence and wellbeing.
Key Responsibilities
- Work with young people with Learning Disabilities transitioning into adult services
- Undertake assessments and support planning in line with relevant legislation
- Work collaboratively with families, carers, education providers, and multi-agency professionals
- Ensure smooth and effective transition planning from children's to adult services
- Support individuals to achieve positive outcomes and independence
Requirements
- Qualified Social Worker with Social Work England registration
- Proven experience working in Learning Disabilities and transitions services
- Strong knowledge of relevant legislation and safeguarding procedures
- Ability to work effectively within a multi-disciplinary team
