We are seeking an experienced and motivated Team Manager to lead a busy Hospital Social Work Team based at Homerton University Hospital in the vibrant London Borough of Hackney. This is an excellent opportunity for a Team Manager with proven management experience within a hospital setting to join a supportive and dynamic service, ensuring the delivery of high-quality, person-centred social care interventions and effective discharge planning.

Key Responsibilities

  • Provide operational leadership and management to a multidisciplinary Hospital Social Work Team.
  • Oversee complex casework, safeguarding concerns, and discharge planning processes.
  • Support, supervise, and develop Social Workers and other team members.
  • Ensure compliance with relevant legislation, policies, and professional standards.
  • Drive service improvements and performance management.
  • Build effective partnerships with health colleagues, community services, and external agencies.

Essential Requirements:

  • Qualified Social Worker with a recognised Social Work Degree
  • Registered with Social Work England.
  • Significant experience working within a Hospital Social Work Team.
  • Previous experience as a Team Manager, Assistant Team Manager, or equivalent leadership role within hospital-based adult social care services.
  • Strong knowledge of Care Act duties, safeguarding, Mental Capacity.
