Eden Brown Synergy is recruiting an experienced Adult Social Worker to join a hospital-based team at Norfolk & Norwich University Hospital. You will support adults during inpatient stays, coordinating with health and social care colleagues to enable safe discharges.
Working with a Home First ethos, you will review individuals after discharge, ensure appropriate support, and contribute to person-centred plans across care settings, travelling as required.
