Occupational Therapist/Physiotherapist – Reablement Team

Hackney, London | £34.34 per hour

Are you an experienced Occupational Therapist/Physio with a passion for helping people regain their independence? Join an exciting new Reablement at Home pilot service with the London Borough of Hackney and play a key role in transforming adult social care.
This innovative therapy‑led service is designed to support adults in achieving person‑centred goals within their own homes, promoting independence, reducing reliance on long‑term care, and facilitating safe, effective hospital discharges.

The Role

As an Occupational Therapist, you will be responsible for delivering high‑quality, strengths‑based reablement interventions that empower individuals to regain confidence and independence in daily living. Working closely with the Integrated Discharge Service, you will assess, monitor, and review therapy‑led reablement programmes, ensuring service users achieve agreed outcomes within a short‑term intervention, typically lasting up to six weeks. You will be based at the Hackney Service Centre, with regular home visits across the borough.

Key Responsibilities

  • Develop and implement person‑centred Reablement Therapy Plans.
  • Identify meaningful goals and outcomes that maximise independence.
  • Monitor progress and review therapy interventions throughout the reablement period.
  • Work collaboratively with Reablement Support Workers, Occupational Therapy Assistants, Senior Practitioners and Adult Social Care teams.
  • Provide professional advice and guidance to reablement providers.
  • Promote a "doing with" rather than "doing for" approach, encouraging service users to regain everyday living skills.
  • Support safe hospital discharge and prevent unnecessary long‑term care packages.
  • Complete accurate documentation and contribute to outcome‑focused reviews.

About the Service

The Reablement at Home service offers up to six weeks of intensive, therapy‑led support to adults eligible for reablement services. The aim is to enable people to live as independently as possible, reduce dependency on ongoing care services, support safe and timely hospital discharges, improve quality of life through strengths‑based interventions and encourage participation in community resources and activities. Deliver care closer to home, at the right time and in the right way. Service users may receive up to four visits per day, seven days a week, with support reducing as independence improves.

Requirements

  • Degree or Diploma in Occupational Therapy.
  • HCPC Registration.
  • Previous experience within Adult Social Care or Community Occupational Therapy.
  • Experience delivering reablement or rehabilitation services.
  • Strong assessment and goal‑setting skills.
  • Excellent communication and multidisciplinary working abilities.
  • Ability to travel throughout Hackney to undertake home visits.
