The predicted salary is between 27 - 40 £ per hour.

This is a community-based Occupational Therapy role with a strong focus on prevention, independence, and reducing long-term reliance on Adult Social Care. You will carry out comprehensive OT assessments for adults with permanent, complex, or substantial disabilities, ensuring interventions promote independence and prevent escalation to ongoing services wherever possible. You will work as part of an integrated, multidisciplinary neighbourhood team, supporting Hackney's strength-based and preventative approach to adult social care.

Key responsibilities:

  • Undertake Care Act‑compliant OT assessments and assessments under relevant housing legislation
  • Assess, prescribe, and review equipment, adaptations, and assistive technology (Telecare)
  • Deliver preventative OT interventions (advice, retraining, equipment, environmental adaptations) to reduce or delay care needs
  • Complete functional assessments to support care planning and packages of care
  • Review existing support packages to ensure they remain appropriate and effective
  • Identify and manage risk associated with equipment and the home environment
  • Contribute to safeguarding enquiries and multi‑agency planning
  • Supervise Occupational Therapy Assistants and OT students where required
  • Maintain accurate and timely records on relevant systems

Requirements:

  • Qualified Occupational Therapist
  • Strong knowledge of Care Act 2014, Mental Capacity Act, and relevant housing legislation
  • Experience working with adults with complex needs in a community setting
  • Commitment to strength‑based, person‑centred practice
