We would prefer staff who work locally due to an office present being needed part of the week. The RIACT team's main roles are to facilitate safe and timely discharges, through use of intermediate care services or long-term services. Following discharge from hospital, it is vital we continue to provide ongoing reviews to support an individual through their recovery process and determining if any longer-term services are required.

As a Social Worker within the RIACT team, it is imperative that you would be able to work alone, whilst also alongside a very hardworking and dedicated team. You will have a good understanding of the Care Act 2014 and hospital discharge processes.
