About the Role:
Eden Brown Synergy is seeking an experienced Social Worker to join the team at Chesterfield Royal Hospital. The ideal candidate will have hospital experience and a strong understanding of the Discharge to Assess (D2A) process. This role involves complex case work, including MCA assessments and safeguarding responsibilities.
Key Requirements:
- Registered Social Worker in England
- Experience in a hospital setting
- Knowledge of Discharge to Assess processes
- Experience with complex case work, MCA, and safeguarding
- Ability to work on-site Monday to Friday, 9 am - 5 pm
