Role Overview: We are currently recruiting for an experienced Speech and Language Therapist to join a community stroke rehabilitation service supporting adult patients across East London. This role focuses on patients with acquired swallowing and communication disorders following stroke and involves predominantly domiciliary visits within the community.

Key Responsibilities:

  • Assess, diagnose, and manage adult patients with swallowing and communication difficulties following stroke
  • Deliver evidence-based dysphagia and communication therapy interventions
  • Complete comprehensive clinical assessments and formulate treatment plans
  • Provide rehabilitation and ongoing support within patients' homes and community settings
  • Work collaboratively within a multidisciplinary team including physiotherapists, occupational therapists, nurses, and consultants
  • Provide education and advice to patients, carers, and families
  • Maintain accurate and timely clinical documentation in line with HCPC and service standards
  • Participate in MDT meetings, discharge planning, and ongoing care reviews

Requirements:

  • HCPC Registered Speech and Language Therapist
  • Experience working with adult stroke patients
  • Competent in dysphagia assessment and management
  • Community experience desirable
  • Ability to work independently and manage a community caseload
  • Full UK driving licence and access to transport desirable

The service is looking for a confident SLT with strong neuro and dysphagia experience who is comfortable working autonomously within a community setting.
