Eden Brown Synergy are currently looking for an experienced Social Worker to join the Step Down Team for Westminster City Council.
Duties and Responsibilities:
- Undertake Care Act assessments and develop support plans following hospital discharge.
- Work within the Discharge to Assess model across homes, rehab units and community settings.
- Complete safeguarding enquiries, Mental Capacity Act assessments and robust risk assessments.
- Manage a varied caseload and ensure timely completion of assessments within set timescales.
- Work collaboratively with health and MDT partners including Homefirst, Reablement and CLCH.
- Promote independence through strengths-based practice and assistive technology.
- Maintain accurate case records and comply with all statutory and council procedures.
- Participate in supervision, CPD, audits and service development activities.

Essential Requirements:
- Social Work Qualification and Social Work England Registration.
- Minimum 1 year post-qualifying experience in a Local Authority or health setting.
- Experience of assessment work, safeguarding and multi-disciplinary working.
- Strong knowledge of Care Act 2014, Mental Capacity Act 2005 and safeguarding legislation.
