Trafford Council are seeking an experienced Social Worker to join their Children with Disabilities (CWD) Team. This is an excellent opportunity to be part of a dedicated service that supports children and young people with complex needs and their families, ensuring they receive the right support at the right time.

The CWD Team in Trafford plays a vital role in assessing needs, delivering support plans, and coordinating multi-agency services to help children and their families achieve positive outcomes.

Key Responsibilities

Manage a caseload of children and young people with disabilities, ensuring their needs are assessed and met in line with statutory requirements
Complete Child and Family Assessments, CIN, CP and LAC work where appropriate
Develop and implement care and support plans tailored to individual needs
Work closely with health, education and partner agencies to coordinate effective multi-disciplinary support
Undertake statutory visits, attend meetings, and contribute to reviews and planning processes
Promote safeguarding and well-being of children and young people at all times
Ensure timely recording and reporting in line with legislation and local procedures

Requirements

Recognised Social Work qualification (Degree or equivalent)
Social Work England registration (current and active)
Experience working within a Children with Disabilities or similar statutory team
Strong understanding of legislation, policy and procedures related to disabled children and safeguarding
Excellent communication, assessment, and multi-agency working skills
Ability to manage a varied and complex caseload effectively
