A well-respected Local Authority in the Caerphilly area is seeking an experienced Qualified Social Worker to join their Community Team. This is an excellent opportunity for a skilled practitioner looking to take on a varied caseload within a supportive and established service. You will play a key role in completing assessments and managing ongoing care arrangements to support adults within the community to live safely and independently.

Key Responsibilities:

  • Undertake strengths-based social care assessments
  • Manage a diverse community-based caseload

  • Support placements in Extra Care Housing, Supported Living, and Residential/Nursing homes
