Bolton Council is recruiting Qualified Social Workers for roles within Intermediate Care and the North Community Mental Health Team. These opportunities are ideal for motivated practitioners with a passion for supporting adults to maximise independence and improve quality of life.
- QSW - Intermediate Care (IMC)
As a Social Worker within Intermediate Care, you will:
- Undertake Care Act 2014 assessments and reviews
- Support care and support planning
- Link individuals into universal and voluntary services
- Support colleagues with complex assessments, Mental Capacity, and Best Interest assessments
- Work closely within an inter-disciplinary team
- QSW - North Community Mental Health Team
This role involves working with adults with mental health needs, undertaking:
- Assessments and reviews under the Care Act 2014
- Support planning to promote independence and wellbeing
- Complex case work including Mental Capacity and Best Interest assessments
- Collaborative working across services and teams
Candidate Requirements (Both Roles):
- Qualified Social Worker (Diploma / Degree in Social Work)
- Passion for working with adults and promoting independence
- Strong relationship-building skills with service users and carers
- Ability to work effectively within multi-disciplinary teams
