We are recruiting for Birmingham City Council in Birmingham for the Social Worker – Transitions Team (Grade 5) position. The role pays £28.40 per hour. Office base: Sparkbrook (every Tuesday). Office attendance is required every Tuesday.
Responsibilities
- You will support young people moving into adult services, completing Care Act assessments, Care Planning, and working closely with families and partner agencies to ensure smooth transitions.
Qualifications
- Qualified Social Worker with SWE registration
- Experience in transitions / adult social care
- Strong assessment and care planning skills
